HAUL’s Health and Wellness Program

Mission Statement
The Houston Area Urban League’s Health and Wellness program aims to increase access to quality healthcare, improve chronic disease management and prevention, and increase access to nutritious foods for under-represented and under-served populations in the greater Houston area through education, awareness and advocacy, health insurance enrollment, faith-based partnerships, and community engagement.

Vision Statement
To transform community health so that every person, regardless of race or income, has an opportunity to achieve optimal health and wellbeing.

1. Access to quality healthcare
a. Access to health insurance
b. Improving health literacy (Target population: people who live in food deserts, areas with high pollution, young people, seniors, neighborhoods with poor health)
2. Chronic disease management and prevention (Target population: neighborhoods with poor health from HHD report, schools, referrals from organizations)
a. Case management
b. Referrals to health services (intra-agency and inter-agency)
c. Education
d. Assessments/triage
3. Access to nutritious foods (Target population: food deserts, low income neighborhoods, early childcare centers, families seeking housing assistance*)
a. SNAP enrollment
b. Referral to local food sources for clients
c. Coordinate student community-service projects to distribute food
d. Intermediary between Houston Food Banka and clients
4. Outcomes
a. Access to HealthCare
b. Chronic Disease
c. Access to Nutritional Foods
